About us

JCU Cares is a locally rooted nonprofit dedicated to improving the wellbeing of the communities it serves. JCU Cares embodies the CBO model by fostering trust, connection, and support among families in the greater Los Angeles area. Through partnerships with health plans, school districts, clinics, and civic organizations, JCU Cares provides culturally responsive programs that address health, housing, education, and social needs—strengthening the family core and empowering individuals to thrive within their community.

Meet Isabel Abramyan: Leadership Rooted in Purpose

 

Isabel Abramyan, founder of  JCU Cares, brings almost 20 years of experience in healthcare, business, and community advocacy. A compassionate and purpose-driven leader, Isabel has dedicated her life to empowering individuals and families to live healthier, more connected lives. Her leadership journey spans both healthcare and community development. As Co-Founder of Jewel City United, Isabel has mentored and guided more than 200 youth, helping to cultivate resilience, leadership, and a sense of belonging. She believes that strong, thriving communities are built by investing in people at every stage of life. Isabel’s deep commitment to holistic well-being inspired the creation of JCU Cares, a mission-driven initiative of Jewel City United focused on prevention, empowerment, and comprehensive wellness for seniors, families, and vulnerable populations. She views health as the harmony of mind, body, and spirit—and is passionate about the transformative power of compassionate, coordinated care. Before founding Jewel City United and JCU Cares, Isabel led CCM Care Group, a Chronic Care Management company, where she worked closely with patients living with chronic conditions to improve their health outcomes and quality of life. She also served as a STAR Measure Coach for Maverick Medical Group, collaborating with healthcare providers and patients to enhance quality performance and increase the organization’s Star Ratings. Further extending her mission of empowerment, Isabel co-founded the Jewel City United Soccer Academy, a program that uses sports as a platform to teach teamwork, discipline, and leadership. The Academy provides a safe and supportive environment where young athletes can build confidence, pursue their goals, and grow both on and off the field. Through her work at JCU Cares and the Jewel City United Soccer Academy, Isabel continues to embody her life’s purpose—uplifting communities through compassion, connection, and care rooted in purpose.
