Enhanced Care Management

 Enhanced Care Management (ECM) is a statewide, no-cost Medi-Cal benefit that became available to eligible members on January 1, 2022. ECM provides a whole-person approach to care, addressing both the medical and social needs of Medi-Cal members through their managed health care plan. 

The goal of ECM is to deliver comprehensive, coordinated support that helps Medi-Cal members achieve better health and well-being by:

• Enhancing care coordination
• Integrating medical, behavioral, and social services
• Connecting members to community-based resources
• Addressing social determinants of health
• Improving overall health outcomes
• Reducing unnecessary or duplicate services

If you qualify for Enhanced Care Management (ECM), you’ll receive support from your own dedicated care team, led by a Lead Care Manager. Your Lead Care Manager will work closely with you and coordinate with your doctors, specialists, pharmacists, case managers, and social service providers to ensure everyone is aligned in supporting your health needs. They’ll also help you connect with community resources and additional services—making it easier for you to get the care, guidance, and support you deserve.

Through Enhanced Care Management (ECM), you’ll receive personalized support designed to meet your unique health and social needs. Services may include:

• Outreach and Engagement – Connecting with you to understand your needs and goals
• Comprehensive Assessment – Evaluating your health, social, and behavioral needs
• Case Management – Creating and managing a personalized care plan
• Enhanced Coordination of Care – Ensuring all your providers work together seamlessly
• Health Promotion and Coaching – Supporting healthy habits and self-management skills
• Comprehensive Transitional Care – Assisting during hospital discharges or care transitions
• Member and Family Support – Helping you and your loved ones navigate your care
• Coordination and Referral to Community and Social Supports – Linking you to local programs and services that can make life easier

Enhanced Care Management (ECM) goes beyond medical care to make sure you have the support you need in all areas of your life. Your care team can help you with:

• Finding the right doctors and scheduling appointments
• Coordinating access to medications and pharmacy services
• Connecting you to transportation, food, housing, mental health, substance use, and other social services
• Providing follow-up support after hospital stays to ensure a smooth recovery

Enhanced Care Management (ECM) is a statewide, no-cost Medi-Cal benefit available to adults and children who are enrolled with a Medi-Cal managed care provider.

You may qualify for ECM if you belong to one or more of the following groups:

• Individuals experiencing homelessness or housing instability
• Individuals with frequent emergency room visits or hospitalizations
• People living with serious mental illness or substance use disorders
• Individuals at risk of entering long-term care (LTC)
• Individuals transitioning out of a nursing facility
• Children and youth involved in the child welfare system
