 JCU Cares provides a Comprehensive Assessment and Care Management Plan designed to meet each individual’s unique needs. Our team works closely with clients to evaluate their physical, emotional, and social well-being, identifying strengths, challenges, and goals. Together, we create a personalized care plan that connects individuals to essential resources, health and social services, and community supports. Through ongoing case management and follow-up, we ensure every client receives coordinated, compassionate, and effective care that promotes long-term stability and self-sufficiency. 

 JCU Cares offers Enhanced Coordination of Care to ensure that individuals and families receive seamless, integrated support across multiple service systems. Our care coordinators collaborate with healthcare providers, social service agencies, and community partners to streamline access to medical, behavioral health, housing, and social resources. By fostering communication and alignment among providers, we help clients navigate complex systems, reduce barriers to care, and achieve better health and life outcomes.

Our Health Promotion services focus on increasing awareness about nutrition, physical activity, mental health, and chronic disease prevention. Through community workshops, outreach events, and one-on-one support, we provide the tools and knowledge people need to make informed health decisions and build lasting habits that enhance overall well-being. .
